The Health History Form gives the physician an overview of you and your life. It enables the physician to notice any potential problems now, or any that may occur in the future.
Preventive medicine is mentioned over and over in today's media. It is important to take care of your health. So grab your pen or pencil, put on your thinking cap, and fill out that form. Who knows, it might save you time in the hospital!
